Hardens React + Vite support: fixes a source-map resolution bug where Vite paths starting with /weren't joined against the map's sourceRoot correctly, adds a bare-filename fallback (deriveFilenameFromBundledUrl) for source-map entries that only give a filename with no directory, and cleans up dead resolution code.

Verified with an automated end-to-end test harness against a real React/Vite project: 11 of 13 realistic error categories were detected. Fix quality is more mixed — 1 of 11 generated fixes was judged a genuine root-cause fix in that run; the rest were safe, guard-clause-level patches. Always review a suggested fix before applying it.

Next.js is not supportedin this release. A misattribution bug tied to Turbopack's sectioned source maps was found and diagnosed, but the fix was reverted before this release — see Known Limitations.
